Instructions for clients of Brigg Noyes, PhD
Before coming in for your first appointment, please complete the following:
- Read through the Client-Therapist Services Agreement and the Notice of Privacy Policies
- Print out and sign the last page of the Client-Therapist Services Agreement (a copy for each person being seen)
- Fill out and bring in a printed copy of the Intake Form for each person being seen
